DHHS caseworkers may refer a child for Heart Gallery photos by completing both the referral form and consent form.  They can be returned by either (1) scanning the completed documents and emailing them to info@secondbestministries.org, (2) faxing to 207-743-7602, or (3) mailing to Second Best Ministries, 64 Rabbit Valley Road, Oxford, ME  04270.  If you have any questions, please call 207-890-0726.
